The Community Support Police Officer for Halliwell will be holding drop in sessions if you wish to discuss anything with him.  He will be here at the surgery on Thursday 21 July 2022 between 10am-12noon and Thursday 18 August 2022 between 10am-12noon. 

 

Andrea, our Social Prescriber Link Worker will also be holding drop in sessions on the same day at the same time if you need support with Mental Health, wellbeing, access to nature, budgeting, support as a carer, social isolation, housing, volunteering or employment, please come and speak to Andrea. 

We have a number of different clinicians working with us, GPs, Practice Nurse, Health Care Assistant, Health Improvement Practitioner, Pharmacist, a Mental Health Practitioner & MSK Practitioner - to help us book you in to see the correct person - Reception will be asking you for your name/dob/tele no & what your appointment is regarding, this is to ensure that 1) you are one of our patients and 2) that you are booked with the correct person which will give them time to prepare for your appointment.

 

We are taking on NEW patients, if you are in our practice are and would like to register, please pop into the surgery for more details, we are on the right as you come through the door!

New Patient Registration

Please Note - 2 forms of identification may be necessary either Utility Bills/Telephone Bills/Bank Statements or some other documentation which shows your address.

Patients ariving from overseas who wish to register are also required to provide proof of permission to stay in addition to proof of address.

If you are on any regular medication, please bring a list of medications. The medication list needs to be in English in order for our doctors to prescribe the correct medication.

We also offer new patient welcome packs in a variety of languages.

Medical treatment is available from the date of registration. Please contact reception for further information.
